How to Register

Welcome to Vine Surgery Partnership

We are pleased to accept registration requests from patients who are living within the area indicated on the map below.

Enter your postcode using the search option to see if you live in our catchment area.

New Patient Registrations

If you live here, or are moving into this area, you are welcome to apply to register with our practice.

Please note: You do not need to register to use Klinik if you are already a patient of Vine Surgery.

To register with us you can either visit the practice and we will give you a registration pack. The pack contains various forms which you can complete at the surgery or take home.

Patient Access If you would like to start using online services for yourself, or if you would like to grant a proxy user, e.g., a carer or relative, to enable you to view your GP records, make appointments and order repeat prescriptions online, please complete the Patient Access forms at the surgery.

Visitors to the Practice

If you are visiting the area and require immediate and necessary treatment, you will need to register as a temporary patient. To register please click to complete the Temporary Services registration online form.

Find your NHS Number

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
